- Uncomplicated lower UTI (cystitis)
- Recurrent UTI prophylaxis

Mechanism of Action
Nitrofurantoin is activated by bacterial nitroreductase to reactive intermediates that damage bacterial DNA, ribosomes, and metabolic enzymes. Concentrates 50x in urine vs serum — making it ineffective for pyelonephritis but excellent for cystitis. Low CRR (collateral resistance).
Dosage & Administration
- Adults: 100 mg SR twice daily x 5–7 days for UTI;
- 50–100 mg at bedtime for prophylaxis.
Side Effects
- Nausea, GI upset (take with food), pulmonary toxicity (acute hypersensitivity + chronic interstitial pneumonitis with long-term use), peripheral neuropathy, hepatotoxicity (rare).
Contraindications
- Renal impairment (CrCl <30 ml/min — accumulates), G6PD deficiency, pregnancy at term (38–42 weeks), infants <1 month, pyelonephritis.
