Therapeutic Indications
- Refractory chronic spontaneous urticaria (after failure of 20 mg/day)
- Severe allergic rhinitis with prominent ocular symptoms
- Dermographism not controlled by standard antihistamine dose
Mechanism of Action
Higher-dose Bilastine produces greater H1 receptor saturation, improving control of histamine-driven wheals and itch in refractory urticaria. The mechanism is the same as the standard dose — selective peripheral H1 antagonism with negligible CNS effects.
Dosage & Administration
- Adults ≥ 18 years: 40 mg (one tablet) once daily for refractory urticaria, one hour before or two hours after food/fruit juice.
- Use only when 20 mg/day has been inadequate.

- Headache, low rate of somnolence, dizziness, dry mouth, fatigue.
- Rare hypersensitivity reactions.
