Mechanism of Action
Cefoperazone is a third-generation cephalosporin with strong gram-negative coverage including Pseudomonas aeruginosa. It binds penicillin-binding proteins to inhibit bacterial cell wall synthesis. Sulbactam (or tazobactam in combinations) is a beta-lactamase inhibitor that extends the spectrum to beta-lactamase-producing strains.
Dosage & Administration
- Adults: 1.5–3 g every 12 hours by IV infusion or IM injection.
- Severe infections: up to 12 g/day in divided doses.
- Adjust dose in hepatic/renal impairment.
- Strictly hospital use.
Side Effects
- Injection site pain, phlebitis, diarrhoea, mild rash.
- Disulfiram-like reaction with alcohol (Cefoperazone N-methylthiotetrazole side chain).
- Rare: hypoprothrombinaemia, hypersensitivity, pseudomembranous colitis.
Contraindications
- Hypersensitivity to cephalosporins.
- Severe penicillin allergy.
- Concurrent alcohol use.
- Severe hepatic impairment (cefoperazone is biliary-excreted).
