Mechanism of Action
Cefpodoxime Proxetil is an oral third-generation cephalosporin prodrug that is hydrolysed in intestinal mucosa to active cefpodoxime. It binds penicillin-binding proteins, inhibiting peptidoglycan cross-linking. Broad spectrum covering gram-negative (H. influenzae, M. catarrhalis, E. coli, Klebsiella) and selected gram-positive (streptococci, MSSA) organisms — including many beta-lactamase-producing strains. Clavulanate inhibits beta-lactamases.
Dosage & Administration
- Children > 5 years: 10 mg/kg/day of cefpodoxime in two divided doses for 7–10 days.
- Reconstitute with supplied water;
- refrigerate after reconstitution.
Side Effects
- Diarrhoea, nausea, vomiting, mild rash, headache.
- Rare: pseudomembranous colitis, anaphylaxis, severe skin reactions.
- Cross-reactivity with penicillin allergy (~5–10%).
- Increased GI upset due to clavulanate.
Contraindications
- Hypersensitivity to cephalosporins.
- Severe penicillin allergy.
- Caution in renal impairment (dose adjustment).
- Caution in GI disease.
- History of cholestatic jaundice with clavulanate.
