About Isoplex-HF Tablet
Isoplex-HF Tablet combines the venous nitrate Isosorbide Dinitrate 20 mg with the arterial vasodilator Hydralazine 37.5 mg. The combination is established in heart failure with reduced ejection fraction — particularly in African-American patients (A-HeFT trial showed mortality benefit).
It is indicated as add-on therapy in heart failure with reduced ejection fraction (HFrEF), in patients intolerant to ACE inhibitors/ARBs, and in chronic refractory heart failure on optimal background therapy.

Mechanism of Action
Isosorbide Dinitrate is converted to nitric oxide in vascular smooth muscle, predominantly venodilating and reducing preload. Hydralazine is a direct arterial vasodilator (mechanism uncertain — possibly through release of nitric oxide and inhibition of inositol triphosphate-induced calcium release). The combination provides balanced reduction in preload and afterload.
