Mechanism of Action
Mycophenolate (mofetil or sodium) is an immunosuppressant — its active metabolite mycophenolic acid (MPA) reversibly inhibits inosine monophosphate dehydrogenase (IMPDH), blocking de novo purine synthesis. Lymphocytes depend on this pathway (no salvage pathway) making MPA selectively immunosuppressive for T and B cells.
Dosage & Administration
Adults: 1–1.5 g twice daily for transplant rejection. 1 g twice daily for lupus nephritis.
Side Effects
Diarrhoea (very common), leucopenia, anaemia, infection risk, GI upset. Long-term: lymphoma risk, skin cancer.
Contraindications & Precautions
Pregnancy (teratogenic — requires effective contraception), hypersensitivity, severe active infection, severe GI disease.
