About Bilastic-AM Tablet

Bilastic-AM Tablet combines three complementary anti-allergy / anti-asthma molecules: Bilastine 20 mg (H1 antihistamine), Montelukast 10 mg (cysteinyl leukotriene receptor antagonist) and Acebrophylline 200 mg (mucolytic + mild bronchodilator). The triple combination is used for patients with concurrent allergic rhinitis and bronchial asthma — the so-called ‘united airway’ phenotype that drives most chronic respiratory allergy in Indian practice.

Bilastine blocks histamine-mediated rhinitis symptoms; Montelukast blocks leukotriene-mediated bronchoconstriction and mucosal swelling; Acebrophylline reduces mucus viscosity, stimulates surfactant production and provides mild bronchodilation. Together they address the inflammatory, bronchospastic and mucus-clearance components of allergic airway disease in a single daily tablet.

Bilastic-AM is manufactured by Trumac Healthcare under WHO-GMP conditions. Available for monopoly-based PCD pharma franchise and third-party manufacturing — a high-margin respiratory/allergy SKU.

Indications

• Allergic rhinitis with concurrent mild-to-moderate asthma
• Persistent allergic asthma with rhinitis (united airway disease)
• Cough-variant asthma with allergic component
• Post-viral airway hyperreactivity with allergic background

Mechanism of Action

Bilastine selectively blocks H1 receptors, suppressing histamine-driven rhinitis and itch. Montelukast antagonises CysLT1 leukotriene receptors on airway smooth muscle and inflammatory cells, reducing bronchoconstriction and eosinophilic inflammation. Acebrophylline is a xanthine derivative + ambroxol salt — providing mucolytic, surfactant-stimulating and mild PDE-inhibitor bronchodilator action.

Dosage & Administration

Adults: 1 tablet once daily in the evening, swallowed whole. Re-evaluate response after 4 weeks. Step down to monotherapy when symptoms stabilise.

Side Effects

Headache, mild GI upset, somnolence (low), behavioural changes (Montelukast — black-box warning for neuropsychiatric effects; counsel patient and monitor). Acebrophylline class: occasional GI upset, palpitations, tremor.

Contraindications & Precautions

Hypersensitivity to any component. Severe hepatic impairment. Concomitant fruit juices (reduce Bilastine absorption). History of suicidal ideation (Montelukast caution). Acute severe asthma exacerbation — not for rescue therapy.
