Mechanism of Action
Apremilast selectively inhibits phosphodiesterase-4 (PDE-4), increasing intracellular cAMP in immune cells. Elevated cAMP suppresses production of TNF-α, IL-23, IL-17 and other pro-inflammatory cytokines while increasing anti-inflammatory IL-10 — modulating the psoriasis cytokine network.
Dosage & Administration
- Titrate from 10 mg over 6 days to maintenance 30 mg twice daily
- Take with or without food
- Reduce dose in severe renal impairment
- Continue 4 months to assess response

- GI upset (diarrhoea, nausea — most common, usually transient)
- Headache
- Weight loss
- Mood disturbance and depression (boxed warning)
- Upper respiratory infections
