Mechanism of Action
Clindamycin binds the 50S bacterial ribosomal subunit, inhibiting protein synthesis. Bacteriostatic but bactericidal at high concentrations. Inhibits bacterial exotoxin production (relevant in toxic shock syndrome and necrotising fasciitis).
Dosage & Administration
- Adults: 300–450 mg four times daily for 7–14 days.
- Severe infections: up to 600 mg four times daily.
- Take with adequate water (avoid oesophageal irritation).
Side Effects
- Diarrhoea (sometimes Clostridioides difficile colitis — counsel patient on red-flag symptoms), nausea, metallic taste, rash.
- Rare: pseudomembranous colitis, severe hepatotoxicity, Stevens-Johnson syndrome.
Contraindications
- Hypersensitivity to lincosamides.
- History of Clostridioides difficile colitis.
- Severe hepatic impairment.
