About Razium-D3 (Tab-in-Cap)
Razium-D3 is a tab-in-cap format with three different tablets — Rabeprazole (PPI), Domperidone (D2 antagonist), and Itopride (D2 antagonist + cholinesterase inhibitor). Comprehensive acid suppression + dual prokinetic for severe refractory GERD.

Mechanism of Action
Rabeprazole irreversibly inhibits gastric H+/K+ ATPase (proton pump) — markedly suppressing both basal and stimulated acid secretion. Faster onset (within 1 hour) and less CYP2C19 dependence than Omeprazole. Domperidone is peripheral D2 receptor antagonist enhancing GI motility, accelerating gastric emptying, increasing LOS tone without crossing BBB. Itopride adds dual D2 antagonism + cholinesterase inhibition for enhanced prokinetic effect.
Dosage & Administration
Adults: 1 capsule once daily before breakfast.
Side Effects
Headache, diarrhoea, abdominal pain. Long-term: B12 deficiency, hypomagnesaemia, fracture risk, C. difficile colitis. Hyperprolactinaemia, QT prolongation (Domperidone — short-term use).

Hypersensitivity. Caution in severe hepatic impairment. Prolactinoma, QT prolongation.
