About Chlorwise-250 Capsule
Chlorwise-250 Capsule contains Chloramphenicol 250 mg — a classical broad-spectrum antibiotic with rare but serious toxicities (aplastic anaemia, Grey baby syndrome). Despite these concerns, chloramphenicol remains useful in specific situations: severe typhoid fever in regions of multi-drug resistance, rickettsial infections, anaerobic CNS infections and selected meningitis cases where alternatives are limited.
Use should be guided strictly by sensitivity data and specialist input. Routine CBC and reticulocyte monitoring is mandatory during therapy. The 250 mg capsule strength is used in adults at 50–75 mg/kg/day in four divided doses.
Chlorwise-250 is manufactured by Trumac Healthcare under WHO-GMP conditions. Available for monopoly-based PCD pharma franchise, government tenders and third-party manufacturing — a specialty antibiotic SKU.
Indications
- Severe typhoid fever (MDR strains)
- Rickettsial infections (typhus, Q fever, RMSF)
- Anaerobic CNS infections (brain abscess)
- Severe haemophilus meningitis (alternative)
- Plague and tularemia (specialist use)
Mechanism of Action
Chloramphenicol binds the 50S ribosomal subunit and inhibits peptidyl transferase, blocking bacterial protein synthesis. Bacteriostatic against most organisms, bactericidal against H. influenzae, S. pneumoniae and N. meningitidis.
Dosage & Administration
Adults: 50–75 mg/kg/day in four divided doses for 7–14 days based on infection. Strict CBC and reticulocyte monitoring required. Specialist use only.
Side Effects
Aplastic anaemia (rare but fatal — idiosyncratic), reversible dose-related bone marrow suppression, GI upset, optic neuritis with prolonged use, Grey baby syndrome in neonates.
Contraindications & Precautions
Hypersensitivity. History of bone marrow suppression. Neonates and pregnant women (risk of Grey baby syndrome). Mild bacterial infections where safer alternatives exist.
