About Ofzac-OR Suspension
Ofzac-OR Suspension is a triple-action paediatric diarrhoea formula — Ofloxacin (bacterial), Ornidazole (protozoal/anaerobic), and Racecadotril (anti-secretory). Particularly useful for severe paediatric diarrhoea with rapid stool reduction needed.

Mechanism of Action
Ofloxacin is a second-generation fluoroquinolone — inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV blocking DNA replication. Active against gram-negative aerobes, atypicals (Chlamydia, Mycoplasma), and many gram-positives. Excellent oral bioavailability (~95%). Metronidazole/Tinidazole/Ornidazole are nitroimidazoles activated by anaerobic/protozoal enzymes — form DNA-damaging nitro radicals. Target anaerobes (Bacteroides, Clostridium), protozoa (Giardia, Entamoeba, Trichomonas). Racecadotril is an enkephalinase inhibitor — prevents breakdown of enkephalins which reduces intestinal secretion (anti-secretory, not anti-motility).
Dosage & Administration
Children: 5–10 ml twice daily.
Side Effects
GI upset, headache, dizziness, photosensitivity, tendon rupture (rare), QT prolongation, peripheral neuropathy, dysglycaemia.
Contraindications & Precautions
Children below 18, pregnancy, lactation, history of FQ tendinopathy, severe hepatic impairment, prolonged QT.
