Mechanism of Action
Ranolazine is a piperazine derivative that inhibits late sodium current in cardiac muscle — reducing sodium-induced calcium overload, improving diastolic function and reducing myocardial oxygen demand. Used for chronic stable angina unresponsive to other anti-anginals.
Dosage & Administration
- Adults: 500 mg twice daily, may increase to 1000 mg twice daily.
Side Effects
- Dizziness, constipation, headache, nausea, QT prolongation (monitor in cardiac patients).
Contraindications
- Severe hepatic impairment, on strong CYP3A4 inhibitors (ketoconazole, clarithromycin), prolonged QT, on QT-prolonging drugs.
