Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV — enzymes essential for DNA replication, transcription, and repair. By preventing supercoiling and chromosome decatenation, ciprofloxacin produces rapid concentration-dependent bactericidal activity with a sustained post-antibiotic effect.
Dosage & Administration
- Adults: 500 mg twice daily for 7–14 days;
- complicated infections may require longer.
- Adjust dose in renal impairment.
- Take with water;
- avoid antacids/iron/calcium/dairy within 2 hours.
Side Effects
- Nausea, diarrhoea, headache, dizziness, taste disturbance.
- Class effects: tendinopathy and tendon rupture (Achilles), QT prolongation, peripheral neuropathy, CNS effects, photosensitivity, dysglycaemia.

- difficile colitis.
- Rare: aortic aneurysm/dissection risk in susceptible elderly patients.
Contraindications
- Hypersensitivity to quinolones.
- Children/adolescents except specific indications.
- Pregnancy and lactation.
- Concomitant tizanidine.
- Tendon disorder history with quinolones.
- Caution: QT-prolonging drugs, epilepsy, myasthenia gravis, antidiabetics, aortic disease.
