Mechanism of Action
Gliclazide is a second-generation sulphonylurea that stimulates insulin release from pancreatic beta cells by closing ATP-sensitive K+ channels. Has additional antioxidant and vascular protective effects. Metformin reduces hepatic glucose production (gluconeogenesis), improves peripheral insulin sensitivity, and reduces intestinal glucose absorption — without causing hypoglycaemia in monotherapy.
Dosage & Administration
- Adults: 1 tablet once or twice daily with meals.
- Adjust based on glycaemic response.
Side Effects
- Hypoglycaemia, weight gain (less with Glimepiride), nausea, mild GI upset.
- Rare hyponatraemia.
- GI upset, lactic acidosis (rare with Metformin), metallic taste.
Contraindications
- Type 1 diabetes, diabetic ketoacidosis, severe hepatic/renal impairment, pregnancy, lactation, sulpha allergy.
- eGFR < 30 ml/min, acute illness with dehydration, recent IV contrast.
