Mechanism of Action
Metoprolol is a cardioselective beta-1 adrenergic antagonist that competitively blocks beta-1 receptors in the myocardium, slowing heart rate, reducing contractility, lowering myocardial oxygen demand and antagonising sympathetic neurohormonal activation in heart failure. The succinate salt with extended-release matrix gives once-daily dosing and smoother plasma curve than tartrate.
Dosage & Administration
- Adults (HFrEF starter): 12.5 mg once daily, double every 2 weeks as tolerated
- Hypertension: 25–100 mg once daily
- Take with or after food
- Do not crush or chew

- Fatigue, bradycardia

- Postural hypotension

Contraindications
- Severe bradycardia (HR < 50)
- Second or third degree AV block
- Decompensated acute HF
