Mechanism of Action
Ceftriaxone is a third-generation cephalosporin with broad gram-negative and gram-positive activity — binds penicillin-binding proteins (PBPs) inhibiting cell wall synthesis. Once-daily dosing due to long half-life (~8 hours). Penetrates CSF effectively making it first-line for meningitis. Tazobactam is a beta-lactamase inhibitor with broader spectrum than sulbactam — protects Ceftriaxone from extended-spectrum beta-lactamases (ESBL).
Dosage & Administration
Adults: 1.125 g twice daily IV.
Side Effects
GI upset, diarrhoea (C. difficile risk), hypersensitivity (cross-reactivity with penicillins ~5%), biliary sludging (especially with calcium-containing IV fluids in neonates), rare haemolytic anaemia.
Contraindications & Precautions
Cephalosporin/penicillin hypersensitivity, neonates with hyperbilirubinaemia, concurrent calcium IV in neonates (Ringer's lactate).
