About Mucofer Injection
Mucofer Injection delivers N-Acetylcysteine 400 mg IV — the standard antidote for paracetamol (acetaminophen) overdose, where it must be started within 8 hours of ingestion for maximum hepatic protection. Also used for contrast-induced nephropathy prevention.
Manufactured by Trumac Healthcare under WHO-GMP. Available for monopoly-based PCD pharma franchise and third-party manufacturing.
Indications
- Paracetamol (acetaminophen) overdose — antidote
- Contrast-induced nephropathy prevention
- Acute respiratory failure with thick mucus
- Acute liver failure (adjunct)
- Mustard gas exposure (selected)
Mechanism of Action
N-Acetylcysteine (NAC) is a mucolytic and glutathione precursor — its thiol group disrupts disulfide bonds in mucus glycoproteins (mucolytic effect) and provides cysteine for hepatic glutathione synthesis (antioxidant effect). Used for thick respiratory mucus, paracetamol overdose (hepatic glutathione restoration), contrast-induced nephropathy prevention, and chronic obstructive pulmonary disease.
Dosage & Administration
Paracetamol overdose: 150 mg/kg loading dose IV in 200 ml D5W over 60 min, then 50 mg/kg in 500 ml over 4 hours, then 100 mg/kg in 1 L over 16 hours.
Side Effects
Nausea, vomiting, diarrhoea, headache, bronchospasm (rare), hypersensitivity reactions. Anaphylactoid reactions during IV infusion (slow rate to manage).
Contraindications & Precautions
Hypersensitivity, active peptic ulcer (relative), severe asthma (caution — may cause bronchospasm).
