Mechanism of Action
Nebivolol's d-isomer provides high beta-1 selectivity, while the l-isomer activates endothelial beta-3 receptors, releasing nitric oxide and producing peripheral vasodilation. The combination gives BP reduction with reduced fatigue, less effect on lipid/glucose profile, and less worsening of exercise tolerance compared to older beta-blockers.
Dosage & Administration
- Hypertension: 5 mg once daily (range 2.5–10 mg)
- Heart failure in elderly: start 1.25 mg, titrate to 10 mg over 6 weeks
- Take with or without food
- Reduce dose in severe renal impairment
