Therapeutic Indications
- Prevention of cerebral vasospasm after aneurysmal subarachnoid haemorrhage
- Adjunct in ischaemic stroke (off-label)
Mechanism of Action
Nimodipine selectively blocks L-type calcium channels in cerebral vascular smooth muscle. Its high lipid solubility allows preferential cerebral vasodilation. In SAH, it prevents the cerebral vasospasm that causes delayed ischaemic neurological deficits — the BRANT trial showed improved outcome.
Dosage & Administration
- Adults: 60 mg (2 tablets) every 4 hours for 21 days
- Start within 96 hours of SAH
- Take with water on empty stomach
- Use under neurology/neurosurgery supervision
