Mechanism of Action
Ramipril is a prodrug hydrolysed to ramiprilat, a potent angiotensin-converting enzyme (ACE) inhibitor. By blocking conversion of angiotensin I to II, it reduces vasoconstriction, aldosterone secretion and sympathetic activation — lowering BP and providing cardio-renal protection (HOPE trial). HCTZ adds thiazide diuretic effect for additive natriuresis and antihypertensive action.
Dosage & Administration
- Adults: 1 tablet once daily
- Start with lower dose and titrate
- Monitor BP, renal function and potassium
- Take with or without food

- Dry cough (common)
- Hyperkalaemia
- Angioedema (rare but serious)
- Hypotension on initiation
- Renal impairment (in bilateral renal artery stenosis)
