Mechanism of Action
Tegoprazan is a potassium-competitive acid blocker. It binds reversibly to the H+/K+-ATPase (the gastric proton pump) at its potassium-binding site and blocks acid secretion. Unlike proton pump inhibitors, it does not need acid activation, so it can act from the first dose and can be taken with or without food. Its effect is also not significantly influenced by CYP2C19 genetic variation, which changes how many patients respond to PPIs.
Dosage & Administration
Prescription only — to be used as directed by a registered medical practitioner. The usual adult dose is one 50 mg tablet once daily, with or without food. Standard courses are 4 weeks for erosive GERD (extendable by another 4 weeks if needed), 4 weeks for non-erosive GERD, and 8 weeks for gastric ulcer. Safety and efficacy in children have not been established.
Side Effects
Tegoprazan was generally well tolerated in clinical trials, with a side-effect profile comparable to PPIs. Reported side effects include headache, diarrhoea, constipation, nausea and abdominal discomfort. Patients should report any persistent or serious effect to their doctor.
Contraindications & Precautions
Do not use in patients with hypersensitivity to tegoprazan, any ingredient of the tablet, or substituted benzimidazoles. Contraindicated with atazanavir, nelfinavir or rilpivirine-containing products, because reduced stomach acid lowers their absorption. Not for use in pregnant or breastfeeding women.
