Mechanism of Action
Glimepiride closes ATP-sensitive K+ channels on pancreatic beta-cell membranes (via the SUR1 receptor subunit), depolarising the cell and triggering calcium-mediated insulin exocytosis. It has lower binding to extra-pancreatic SUR2 isoforms compared to glibenclamide, giving a better cardiovascular profile.
Dosage & Administration
- Adults: 1 tablet once daily, immediately before breakfast
- Titrate gradually at 1–2 week intervals based on glycaemia
- Maximum 8 mg/day
