Mechanism of Action
Ketoconazole inhibits fungal lanosterol 14-alpha-demethylase (CYP51) — blocking ergosterol synthesis. Also inhibits human CYP enzymes including CYP3A4 (drug interactions) and CYP17 (suppressing adrenal/gonadal steroidogenesis at high doses).
Dosage & Administration
- Adults: 200 mg once daily for 7–14 days.
- Avoid prolonged use due to hepatotoxicity.
Side Effects
- Hepatotoxicity (monitor LFTs), GI upset, gynaecomastia, decreased libido, adrenal suppression at high doses.
Contraindications
- Hepatic disease, on hepatotoxic drugs, pregnancy, CYP3A4 substrate drug interactions (statins, warfarin, BZDs etc).
