Mechanism of Action
Midodrine is a prodrug rapidly converted to its active metabolite desglymidodrine, a selective alpha-1 adrenergic agonist. Activation of vascular alpha-1 receptors produces arteriolar and venous vasoconstriction, raising peripheral resistance and central venous return — thus increasing BP.
Dosage & Administration
- Adults: 2.5–10 mg three times daily
- Take during daytime only (last dose by late afternoon)
- Never take within 4 hours of bedtime (supine hypertension risk)
- Monitor supine and seated BP

- Supine hypertension (very common — biggest concern)
- Pilomotor reaction (goose bumps)
- Scalp pruritus
- Urinary urgency
- Paraesthesias
Contraindications
- Severe organic heart disease
- Acute renal disease
- Urinary retention
- Phaeochromocytoma
- Thyrotoxicosis
- Supine systolic BP > 180 mmHg
- Lying-down dosing (use only when upright)
