Mechanism of Action
Acenocoumarol competitively inhibits vitamin K epoxide reductase, preventing regeneration of active vitamin K. This blocks the gamma-carboxylation of vitamin-K-dependent clotting factors II, VII, IX and X — reducing their functional activity and producing anticoagulation.
Dosage & Administration
- Adults: titrate dose by INR (target 2.0-3.0 for most indications, 2.5-3.5 for mechanical valves)
- Typical maintenance: 1-8 mg daily
- Take at same time each day
- Regular INR monitoring mandatory
