Dosage & Administration
- Adults: 15,000–25,000 IU/kg/day in divided doses every 12 hours.
- Loading dose 25,000 IU/kg.

- Nephrotoxicity (dose-dependent — monitor creatinine), neurotoxicity (paraesthesia, neuromuscular blockade), hypersensitivity, bronchospasm with nebulised use.
Contraindications
