About Bilastic-M Suspension
Bilastic-M Suspension combines Bilastine 4 mg/ml with Montelukast 10 mg/ml in a paediatric-friendly suspension. The combination simultaneously addresses the histaminergic and leukotriene-mediated arms of allergic disease — particularly valuable in children with concurrent allergic rhinitis and mild asthma (the so-called united airway phenotype).
Bilastine provides selective H1 receptor antagonism with the lowest sedation profile among second-generation antihistamines. Montelukast blocks the CysLT1 leukotriene receptor on airway smooth muscle and inflammatory cells, reducing bronchoconstriction, mucosal oedema and eosinophil recruitment. The combination is supported by GINA and ARIA guidelines for children with concurrent rhinitis and asthma.
Bilastic-M Suspension is manufactured under WHO-GMP conditions. Available for monopoly-based PCD pharma franchise and third-party manufacturing — a high-volume paediatric anti-allergy SKU.
Indications
- Allergic rhinitis with concurrent mild asthma in children
- Persistent allergic asthma with significant rhinitis
- Exercise-induced bronchospasm (paediatric — montelukast component)
- Aspirin-exacerbated respiratory disease (specialist use)
Mechanism of Action
Bilastine selectively blocks peripheral H1 histamine receptors. Montelukast competitively binds the CysLT1 leukotriene receptor on airway smooth muscle, eosinophils and mast cells, preventing leukotriene-induced bronchoconstriction, mucosal swelling and inflammatory cell recruitment.
Dosage & Administration
Children 6 months to 5 years: 5 ml once daily in the evening. Children 6–14 years: 5–10 ml once daily. Shake well before use. Take preferably one hour before or two hours after food.

Contraindications & Precautions
Hypersensitivity to either component. Acute asthma exacerbation (not rescue). Severe hepatic impairment. History of neuropsychiatric disorder (Montelukast caution).
