About Clomac Cream

Clomac Cream contains Clobetasol Propionate 0.05% — the most potent topical corticosteroid in clinical use (Class I, ultra-high potency). It is used for short-term treatment of severe, recalcitrant dermatoses including severe atopic eczema, psoriasis, discoid lupus erythematosus, lichen planus and severe contact dermatitis when less potent steroids have failed.

Maximum 2 weeks at a time, maximum 50 g per week, avoid face/groin/flexures, never under occlusion. Rebound flares are common after stopping; tapering to lower-potency steroid is recommended. The risk-benefit balance favours its short-term use only in genuinely severe disease.

Mechanism of Action

Clobetasol is a fluorinated glucocorticoid with very high glucocorticoid receptor affinity. It potently suppresses inflammatory gene transcription, immune cell infiltration and inflammatory cytokine production in the skin. The ultra-high potency comes at the cost of significantly higher systemic absorption and local atrophy risks compared with less potent steroids.

Dosage & Administration

Apply a thin layer to affected area once or twice daily. Maximum 2 weeks at a time, maximum 50 g per week. Do not use under occlusion. Avoid face, groin and flexures.

Side Effects

Skin atrophy, telangiectasia, striae, perioral dermatitis, acneiform eruptions, hypertrichosis. Systemic absorption can cause HPA-axis suppression (especially with prolonged use or large surface area). Tinea incognito if used on undiagnosed fungal infection.

Contraindications & Precautions

Hypersensitivity. Viral skin infections (herpes simplex, varicella). Bacterial or fungal skin infections (untreated). Rosacea, acne, perioral dermatitis. Use on face/groin/flexures (relative contraindication). Children (limit duration and area).
