Therapeutic Indications
- Chronic heart failure with reduced ejection fraction (HFrEF)
- HFrEF NYHA class II–IV
- HF with reduced ejection fraction unsuitable for further ACE inhibitor
Mechanism of Action
Sacubitril is a prodrug for the active neprilysin inhibitor LBQ657, which increases natriuretic peptide levels (ANP, BNP) — promoting natriuresis, vasodilation and reduced cardiac remodelling. Valsartan provides standard AT1 receptor antagonism. The combined ARNI mechanism gives superior heart-failure outcomes vs ACE inhibitors alone.
Dosage & Administration
- Start at lower dose (Sacuvon-50) twice daily
- Double every 2–4 weeks to target Sacuvon-200 twice daily
- Stop ACE inhibitor at least 36 hours before starting
- Take with or without food
- Use under cardiologist supervision

- Hypotension
- Hyperkalaemia
- Renal impairment
- Angioedema (rare)
- Cough (less than ACE inhibitors)
Contraindications
- Concurrent ACE inhibitor (36-hour washout required)
- History of angioedema
- Pregnancy and breastfeeding
- Severe hepatic impairment
- Bilateral renal artery stenosis
