About Cefiza-O Dry Syrup
Cefiza-O Dry Syrup combines Cefixime 50 mg with Ofloxacin 50 mg per 5 ml in a paediatric dry-syrup format. Used for severe paediatric infections with mixed-flora suspicion or where mono-therapy has been ineffective — particularly under specialist supervision.
The combination broadens spectrum but use in children requires specialist judgment due to fluoroquinolone-related concerns regarding cartilage development. Reserved for situations where benefit clearly outweighs risk.
Cefiza-O Dry Syrup is manufactured by Trumac Healthcare under WHO-GMP conditions. Available for monopoly-based PCD pharma franchise and third-party manufacturing.
Indications
- Acute pharyngitis and tonsillitis (streptococcal)
- Acute otitis media
- Acute exacerbations of chronic bronchitis
- Uncomplicated UTI and cystitis
- Uncomplicated gonorrhoea (single dose)
- Typhoid fever (uncomplicated)
- Paediatric mixed-flora infections (specialist use)
- Severe diarrhoea with bacterial component
Mechanism of Action
Cefixime binds penicillin-binding proteins (PBP-3) on gram-negative bacteria, inhibiting the final transpeptidation step of peptidoglycan synthesis. The result is bacterial cell wall destabilisation and osmotic lysis. Cefixime is resistant to many beta-lactamases produced by gram-negative organisms — distinguishing it from earlier cephalosporins. Ofloxacin inhibits bacterial DNA gyrase.
Dosage & Administration
Children: as advised by paediatrician. Reconstitute with supplied water and shake well.
Side Effects
Diarrhoea (relatively common), nausea, abdominal pain, mild skin rash. Less common: hypersensitivity reactions, transient elevation of liver enzymes. Rare: pseudomembranous colitis, severe skin reactions, anaphylaxis. Fluoroquinolone concerns: theoretical cartilage effects (limit duration), tendinopathy (very rare in children at standard doses).
Contraindications & Precautions
Hypersensitivity to cephalosporins. Severe penicillin allergy (cross-reactivity ~10%). Renal impairment requires dose adjustment. History of GI disease (caution). Tendon disorder history. Quinolone hypersensitivity.
Storage
Dry powder below 30 °C. After reconstitution, refrigerate and use within 7 days.
