Mechanism of Action
Albendazole binds beta-tubulin of helminth cells, preventing microtubule polymerisation; this blocks glucose uptake and ATP synthesis, leading to parasite death. Ivermectin selectively binds glutamate-gated chloride channels in nematode and arthropod nervous systems, causing increased chloride permeability, hyperpolarisation, paralysis and death. Mammalian cells lack these channels, conferring selectivity.
Dosage & Administration
- Children > 2 years: 10 ml as a single oral dose;
- may repeat after 2 weeks for threadworm.
- Adolescents and adults: as directed by physician.
- Take with food.
- Shake the bottle well before each use.
Side Effects
- Mild abdominal pain, nausea, diarrhoea, headache, dizziness.
- Rare: skin rash, transient hepatic enzyme elevation, leucopenia with prolonged albendazole use.
- Mazzotti reaction in onchocerciasis (not common with single-dose use).
Contraindications
- Hypersensitivity to albendazole, ivermectin or any benzimidazole.
- Pregnancy (first trimester).
- Children under 2 years unless prescribed.
- Caution in hepatic impairment and concurrent CNS infections (Loa loa).
