Mechanism of Action
Amikacin binds irreversibly to the 30S bacterial ribosomal subunit, causing misreading of mRNA and inhibition of protein synthesis. The resulting production of non-functional proteins damages bacterial cell membranes and leads to rapid bactericidal effect. Aminoglycosides exhibit concentration-dependent killing and a post-antibiotic effect — supporting once-daily dosing strategies.
Dosage & Administration
- Adults: 15 mg/kg/day, given as a single daily dose or in two divided doses by IM/slow IV infusion.
- Adjust dose in renal impairment based on serum levels.
- Maximum duration generally 7–10 days.
- Strictly under medical supervision.
Side Effects
- Nephrotoxicity (acute tubular necrosis), ototoxicity (both vestibular and cochlear), neuromuscular blockade.
- Local injection-site pain, transient elevation of liver enzymes.
- Rare: hypersensitivity, rash, blood dyscrasias.
Contraindications
- Known hypersensitivity to aminoglycosides.
- Concomitant nephrotoxic or ototoxic drugs (furosemide, vancomycin, cisplatin, ciclosporin) without monitoring.
- Pre-existing 8th cranial nerve damage.
- Myasthenia gravis.
- Pregnancy (Category D — risk of fetal ototoxicity).
- Therapeutic drug monitoring strongly advised.
