Mechanism of Action
Clarithromycin is a 14-membered macrolide that binds the 50S bacterial ribosomal subunit, inhibiting peptidyl transferase and protein synthesis. Bacteriostatic against most organisms but bactericidal at high concentrations against susceptible species. Active against most respiratory pathogens including atypicals (Mycoplasma, Chlamydia, Legionella) and H. pylori.
Dosage & Administration
- Adults: 250 mg twice daily for 7–14 days.

Side Effects
- Taste disturbance (metallic taste — characteristic), nausea, diarrhoea, abdominal pain, headache.
- QT prolongation (avoid with other QT-prolonging drugs).

- Rare: hepatotoxicity, hypersensitivity.
Contraindications
- Hypersensitivity to macrolides.
- Severe hepatic impairment.
- Concurrent ergot alkaloids, pimozide, cisapride, lovastatin/simvastatin (severe interactions).
- History of QT prolongation.
- Caution in renal impairment.
