Therapeutic Indications

Mechanism of Action
Budesonide binds intracellular glucocorticoid receptors in airway epithelial and inflammatory cells. The activated receptor complex enters the nucleus and modulates gene transcription — increasing anti-inflammatory protein expression (annexin-1, IκB) and suppressing pro-inflammatory gene expression (cytokines, chemokines, COX-2). This reduces eosinophil and lymphocyte recruitment, mucosal oedema and bronchial hyperreactivity.
Dosage & Administration
- Adults: 1 respule (0.5 mg) twice daily by nebulisation;
- maximum 2 mg/day.
- Children 6 months–12 years: 0.25–0.5 mg twice daily.
- Croup: single 2 mg dose by nebulisation.
- Use a jet or ultrasonic nebuliser with mouthpiece or face mask.
- Rinse mouth after use.
Side Effects
- Local: oral candidiasis (rinse mouth after use to prevent), dysphonia, throat irritation.
- Systemic (rare at standard doses): adrenal suppression, growth retardation (prolonged paediatric use at high doses), cataracts, decreased bone density.
- Rare: hypersensitivity, paradoxical bronchospasm.
Contraindications
- Hypersensitivity to budesonide.
- Active untreated respiratory tract infection (use only under medical supervision).
- Hypersensitivity to milk proteins (some formulations).
- Severe paradoxical bronchospasm.
Storage
Store below 25 °C in a dry place, protected from light. Use immediately after opening the foil sachet.
