Mechanism of Action
Mycophenolate (mofetil or sodium) is an immunosuppressant — its active metabolite mycophenolic acid (MPA) reversibly inhibits inosine monophosphate dehydrogenase (IMPDH), blocking de novo purine synthesis. Lymphocytes depend on this pathway (no salvage pathway) making MPA selectively immunosuppressive for T and B cells.
Dosage & Administration
- Adults: 720 mg twice daily (2 tablets of 360 mg twice daily) — equivalent to 1 g MMF twice daily.
Side Effects
- Diarrhoea (very common), leucopenia, anaemia, infection risk, GI upset.
- Long-term: lymphoma risk, skin cancer.
- Better GI tolerance than MMF.
Contraindications
- Pregnancy (teratogenic — requires effective contraception), hypersensitivity, severe active infection, severe GI disease.
