Mechanism of Action
Prasugrel is metabolised to its active thiol metabolite which irreversibly binds the P2Y12 ADP receptor on platelets, blocking ADP-mediated platelet activation. More efficient hepatic activation than clopidogrel gives faster, stronger, more consistent inhibition (less inter-patient variability).
Dosage & Administration
- Loading dose: 60 mg orally as single dose at time of PCI
- Maintenance: 10 mg once daily with aspirin
- Use under cardiologist supervision
- Maintain dual antiplatelet for at least 12 months post-stent (or as advised)
Side Effects
- Bleeding (major and minor; higher rate than clopidogrel)
- Bruising
- Anaemia
- Thrombotic thrombocytopenic purpura (rare)
- Hypersensitivity
Contraindications
- History of TIA or stroke
- Active pathological bleeding
- Severe hepatic impairment
- Age > 75 years (avoid)
- Weight < 60 kg (use reduced dose 5 mg)
