About Amiwise-100 Tablet
Amiwise-100 Tablet contains Amisulpride 100 mg, an atypical antipsychotic with a unique pharmacology — it selectively binds dopamine D2 and D3 receptors, with little affinity for serotonin, histaminic, muscarinic or adrenergic receptors. This selectivity gives it a favourable side-effect profile compared with older typical antipsychotics.
At low doses (50–300 mg/day), Amisulpride preferentially blocks presynaptic D2/D3 autoreceptors, enhancing dopaminergic transmission — making it particularly effective for the negative symptoms of schizophrenia (apathy, social withdrawal, blunted affect). At higher doses (400–800 mg/day) it blocks postsynaptic receptors to control positive symptoms (hallucinations, delusions).
Amiwise-100 is manufactured by Trumac Healthcare under WHO-GMP conditions and is available for monopoly-based PCD pharma franchise and third-party manufacturing — a specialty psychiatry SKU.
Indications
- Acute and chronic schizophrenia (positive and negative symptoms)
- Schizophrenia with predominantly negative symptoms (low-dose)
- Dysthymic disorder (specialist use)
- Schizoaffective disorder (adjunct)
Mechanism of Action
Amisulpride selectively antagonises post-synaptic dopamine D2 and D3 receptors in the mesolimbic system, controlling positive psychotic symptoms. At lower doses it preferentially blocks presynaptic autoreceptors in the mesocortical pathway, raising synaptic dopamine and improving negative symptoms. Minimal binding to non-dopaminergic receptors limits adverse effects.
Dosage & Administration
Predominantly negative symptoms: 50–300 mg/day in 1–2 divided doses. Mixed positive/negative symptoms: 400–800 mg/day in two doses; maximum 1200 mg/day. Reduce dose in renal impairment. To be prescribed by a psychiatrist.
Side Effects
Insomnia, anxiety, agitation, somnolence, weight gain, hyperprolactinaemia (galactorrhoea, amenorrhoea, sexual dysfunction). Extrapyramidal symptoms at higher doses. Important: dose-dependent QT prolongation — ECG monitoring recommended.
Contraindications & Precautions
Hypersensitivity to amisulpride. Prolactin-dependent tumours (prolactinoma, breast cancer). Phaeochromocytoma. Concomitant levodopa or other dopaminergic drugs. Severe renal impairment. Long QT syndrome or risk thereof. Children under 15 years.
Storage
Store below 30 °C in a dry place, protected from light and moisture.
