About Brivamac-50 Tablet

Brivamac-50 Tablet contains Brivaracetam 50 mg, a third-generation antiepileptic and selective high-affinity SV2A (synaptic vesicle protein 2A) ligand. It is approved as adjunctive therapy and as monotherapy in adults and adolescents (≥ 16 years) with focal-onset seizures, with or without secondary generalisation.

Brivaracetam binds SV2A with ~20-fold higher affinity than levetiracetam (its predecessor), achieving faster onset of seizure control, lower discontinuation rates due to behavioural side effects, and better tolerability — particularly in patients who experienced irritability or psychiatric symptoms on levetiracetam. The 50 mg twice-daily strength is the standard maintenance dose.

Mechanism of Action

Brivaracetam is a selective, high-affinity ligand of synaptic vesicle protein SV2A in the presynaptic terminal. Binding SV2A modulates neurotransmitter release in epileptic foci, reducing the abnormal hypersynchronous firing that underlies focal seizures. Unlike older antiepileptics, it does not act primarily through sodium-channel, GABA or glutamate receptors.

Dosage & Administration

Adults: starting dose 50 mg twice daily; may be adjusted to 25–100 mg twice daily based on response and tolerability. Dose adjustment in hepatic impairment. May be taken with or without food. Can be initiated or discontinued without titration (advantage over many AEDs).

Side Effects

Drowsiness, dizziness, fatigue, headache, nausea, vomiting. Less commonly: behavioural disturbances (less than levetiracetam), bronchospasm, angioedema (rare).

Hypersensitivity to brivaracetam, levetiracetam or any pyrrolidone derivative. Caution in hepatic impairment, psychiatric history, and in patients on sedating medications. Pregnancy — limited data; use only if essential.
